The purpose of this paper is to describe an evidence-based proposal project. Falls and falls with injuries remains the most costly of adverse events. While no one can completely prevent a fall from happening, there is the potential to accurately assess the patient’s risk level for falls. Using Hester Davis Scale for Falls Risk Assessment (HDS) can potentially decrease the rate of falls, but especially reduce the rate for falls with injury. Working closely with the Hester Davis team to successfully implement the HDS, Kaiser Permanente (KP) Southern California Region intends to see a decrease in all falls and especially falls with injury. This paper will discuss the plan for implementation, present literature to support the changes and give more information, as needed. The project is named Hester Davis at Kaiser Permanente. The sponsor of the project is the Chief Nurse Executive of KP, Downey, Nanette Antonio. Leads are Stella Kientz and Alaine Lounsbury. The co-lead is Lori Jaramillo. 2

EBP Proposal – Section A – G Evidence-Based Practice – Organizational Culture and Readiness Assessment Prior to implementing evidence-based practice within an organization, one must first complete an Organizational Culture and Readiness Assessment (Melynyk & Fineout-Overholt, 2011, p. 594). The results will advise of the organization’s culture and readiness for evidence- based practice. Organizational Readiness Based on the results of the Organizational Culture and Readiness Assessment, the organization is ready for evidence-based practice. Out of the eighteen questions, the majority of the responses were four and five with only two responses being three. Being that evidence-based practice (EBP) and research is one of the organization’s pillars of work for nurses, the organization has been moving this direction for quite some time. EBP is ingrained in the organization’s nursing professional practice and vision. The nursing staff is committed to driving evidence-based practice. Physicians within the organization are committed to and support nurses in evidence-based practice. At the regional level, there is a nursing research department with several dedicated nurse researchers/scientists that have work that is published in many journals. At a national level, the leaders support evidence-based practice, as well, through providing education hours to the staff. Not only does the organization support and encourage evidence-based nursing practice, it provides many opportunities for their staff to learn and practice EBP, as well. There are many advance practice nurses within the organization that help translate and teach EBP. As stated above, there is a nursing research department that mentors a few nurses each year.
